Please click on the three forms below, (Medical History (2 pages), Registration and HIPAA (sign page 3) print them out, complete and bring with you the day of your appointment along with your insurance card and a list of current medications you are taking.  Don't forget!!  If your insurance requires you to have a written referral from your primary care provider, make sure you bring it with you.
Having these forms completed before you come to the office will speed up the registration process.

Dear New Patient:

On the day of your appointment, please report to the second floor room #203, 15 minutes before your appointment time.

If you have managed care insurance plan and you need an insurance referral from your primary care doctor, please make sure you bring it with you.  If you cannot obtain the appropriate referral, we must reschedule your appointment.  Please don't let your primary doctor FAX the referral to our office.  It has been our experience that these FAXED referrals do not get to our office forcing us to reschedule your appointment.

Please remember to bring with you: 

  1. Insurance Card and a Picture I.D. (either drivers license or passport) and your co-pay, if required.
  2. Any recent Chest X-Rays or Chest CAT scan films with the written reports. (If you are coming for breathing problems)
  3. A complete list of all current medications you are taking.

If you cannot make the scheduled appointment, please notify us as soon as possible so we can use the valuable appointment space for another.  We reserve the right to charge a fee to New Patients who fail their appointment.
